Job Description

  • Provides for regular Quality Improvement meetings for the review, revision, and improvement and ongoing measurement of Hospice and Home Health quality initiatives.

    Implements and monitors compliance with agency policies and procedures.

    Serves as a mentor and role model.

    Collaborates with Director of Clinical Operations to provide staff meetings for information and staff support as indicated.

    Makes visits to patient/families as indicated by patient/family need and staff need for the purpose of supervision and/or direct nursing care.

    Participates in the supervision of the maintenance of medical records, assuring accuracy, completeness and compliance with licensing regulations, certification standards, legal and ethical standards.

    Promotes the utilization of resources for the benefit of team members, patients and families consistent with agency procedures.

    Identifies need for staff education and assistance in the development of inservice programs for interdisciplinary team.

    Demonstrates knowledge of nursing practice.

    Develops and implements policies and procedures that guide and support the provision of services.

    Continuously assesses and improve the agencies performance.

    Maintain quality improvement programs, as appropriate.

    Supports agency's philosophy, goals, objectives, mission, vision and values.

    Participates in quality improvement to improve processes and patient care.

    Schedules support staff to participate in quality improvement initiatives.

    Implements actions, as appropriate, recommended by quality improvement initiatives.

  • Accept other assignments as may be appropriate.

Requirements

  • A registered professional nurse with a current license to practice in the State of New Mexico.

    Demonstrates knowledge regarding Medicare/Medicaid Hospice and Home Health regulations.

    At least one year of nursing experience in a community health service setting within the past six years.

    At least one year experience in Quality Assurance

    Demonstrated ability to manage all aspects of terminal care and home health care with specific expertise in crisis intervention, family counseling, pain and symptom control and bedside nursing.

    Demonstrated skill in administrative performance which includes expertise with communication, both written and oral, supervision of staff, documentation requirements, and service coordination.

    Demonstrated ability to execute a supportive, sensitive approach to staff and patient/family needs.
