Job Description

·         Participate in the development of the patient/family plan of care.

·         Make home visits to evaluate patient’s level of function through the use of diagnostic and prognostic procedures.

·         Develop a specialized plan of care based on the evaluation and on the goals recommended by the interdisciplinary team and agreed to by the patient and attending physician.

·         Provide direct implementation of treatment or instruction of other members of the health care team, including family members and attendants in the therapy.

·         Consult with agency’s professional staff regarding patient functioning activities and plan of care.

·         Reassess clinical signs and symptoms to determine effectiveness of treatment program.

·         Advise and instruct patient, family and agency staff regarding participation in the treatment program.

·         Observe and report to the Director or Nurse Case Manager the patient’s reaction to the occupational therapy regime and any related changes in the patient’s condition.

·         Participate in the interdisciplinary team conferences as necessary.

·         Document care and services provided on agency’s progress record and consultation forms within time frame consistent with agency’s policy.

·         Take part in agency’s in-service education programs.

·         Demonstrates understanding of CQI process.

·         Actively participates in CQI.

·         Practices and actions reflect the CQI recommendations and organization-wide policies and procedures.

·         Takes responsibility for maintaining accountability in job performance and for improving job knowledge and skills.

·         Supports agency’s philosophy, goals, objectives, mission, vision and values.

·         Accept other assignments as appropriate.

Requirements

·         Graduate of an occupational therapy curriculum accredited by the Council of Medical Education of the American Medical Association and the American Occupational Therapy Association.

·         Licensed by the State of New Mexico.

·         Comprehensive knowledge of the principles, concepts and methodology required of an occupational therapist and its practice application in the treatment of hospice patients at home.

·         Communication and documentation skills.

·         Demonstrated ability to work with a interdisciplinary team.

·         New Mexico driver’s license and availability of a car with insurance per state requirement.
