Job Description

·         Participate in the development of the patient/family plan of care.

·         Make home visits to evaluate patients presenting problems and perform appropriate examination utilizing specialized evaluation expertise.

·         Develop a specialized care plan based on the evaluation and on the goals recommended by the interdisciplinary team and agreed to by the patient and attending physician.

·         Implement the treatment program through direct treatment of the patient.

·         Reassess clinical signs and symptoms to determine effectiveness of program.

·         Advise and instruct patient, family and Advantage Home Care staff regarding participation in the treatment program.

·         Observe and report the Director or the Nurse Case Manager the patient’s reaction to the physical therapy regimen and any related changes in the patient’s condition.

·         Participate in the Interdisciplinary Team conference, as necessary.

·         Supervise assistants, providing continuous and in close proximity supervision to the assistant performing the related tasks. Must be readily available, at all times, to provide advice or instruction to the assistant.

·         Directing or supervising physical therapist shall notify the Board in writing of the commencement of the relationship and shall assume full responsibility for the professional activities of the assistant.

·         The directing or supervising physical therapist shall provide the Board with written notification of the termination of this relationship.

·         The physical therapist shall not be responsible for the direction or supervision of more than 2 assistants.

·         The direction and supervision of the physical therapy assistant(s) shall be the following:

·         The referring physical therapist is responsible for the patient’s care.

·         The supervising physical therapist shall be on call and readily available within a 100 mile radius or appoint another physical therapist in his/her absence.

·         A current written plan of care shall be formulated for each patient by the referring physical therapist and shall be revised following periodic re-evaluations.

·         It is the sole responsibility of the directing or supervising physical therapist to ensure safety of the patient and the quality of care rendered.

·         Document care and services provided on agency’s record and consultation forms within the time frame consistent with the agency’s policy.

Requirements

·         Graduate of an accredited physical therapy curriculum approved by the American Physical

·         Therapy Association or other approving body, with a baccalaureate or master’s degree and certified in physical therapy.

·         Licensed by the State of New Mexico.

·         Comprehensive knowledge of the principles, concepts and methodology required of a physical therapist and its practice application in the treatment of patients at home.

·         Communication and documentation skills.

·         Demonstrated ability to work with an interdisciplinary team.

·         New Mexico driver’s license and availability of a car with insurance per state requirement.
